Sometimes, a patient’s survival depends on traveling thousands of kilometers: from one country to another, to reach the right medical facility at the right time. Medical Evacuation is a demanding yet deeply meaningful field that blends clinical precision, international coordination, and human compassion.
Recently, dr. I Gede Sadu Pratama Werdi, one of our emergency physicians, successfully escorted a patient from Indonesia to Tokyo for advanced medical care. Prior to departure, a comprehensive pre-flight medical assessment was conducted to evaluate the patient’s fitness for commercial air travel, in accordance with international aviation medical considerations and destination country regulations. Coordination was also established with the patient’s family, airline representatives, and relevant diplomatic and hospital contacts to ensure regulatory compliance and smooth cross-border handling.
During the flight, Dr. Sadu provided continuous medical supervision, carrying essential portable medical equipment, including a vital sign monitor and emergency medications. The cabin crew was thoroughly briefed before departure regarding the patient’s condition, potential risks during the flight, contingency plans, and the specific assistance that might be required from the crew if the patient’s condition changed.

Throughout the journey, close attention was paid not only to the patient’s vital signs, but also to mental status, comfort, and communication patterns, allowing early recognition of any clinical or psychological changes. Open and calm communication with the patient helped maintain stability and trust during the flight.
Clear and consistent communication was maintained between the escort team, the patient’s family, airline staff, embassy representatives, and the receiving medical facility in Tokyo. Upon arrival, the patient was safely handed over to the local medical team, followed by a structured explanation to the family regarding the patient’s condition, immediate next steps, and ongoing care plan.
This mission reflects that a medical escort is not simply about transporting a patient from one location to another, but about preserving continuity of care throughout the entire journey, from departure to final handover.

Not all evacuations happen in the air.
Our team also handled ground operations for a medical escort bound for Amsterdam, led by Dr. I Putu Gita Prasetya Adiguna. This critical part of the evacuation process is known as ground handling.
Ground handling in medical evacuation refers to the complete coordination of patient movement on the ground, both before and after the flight. This includes medical supervision during transfers between the hospital and airport, ambulance standby at departure and arrival points, coordination with airport authorities and customs, and ensuring that all medical equipment and oxygen supplies are properly cleared for transport.
Ground handling bridges the gap between air and land, ensuring that the patient’s journey is seamless, medically safe, and logistically smooth from hospital bed to aircraft seat, and back to hospital bed at the destination. Dr. Gita and the team worked closely with airport authorities, airline crews, to ensure a smooth and safe transition. Every step, from documentation to on-site care, was guided by one central principle: zero compromise on patient safety. Such missions highlight the importance of having a medical team trained not only in emergency care but also in the intricate logistics of international patient transfer.
